Number In Party Banquet Seating: If there are others who are registering separately from you with whom you would like to be seated at the banquet, please list their names below. While every attempt will be made to seat everyone with whom they request, space and timing may prevent us from being successful.

Optional Tour: If you wish to take the opportunity to tour fabulous Los Angeles by motor coach on Friday, April 3rd, please indicate below the number of tickets you would like. The tour takes approximately four hours and the bus will depart the Queen Mary hotel at 9 am. Tickets are $35.00 each

Reserve (how many) ticket(s) $35.00 per ticket.

Confirmation: Your credit card statement will be your confirmation. Nothing will be mailed back to you. You and your guests must each bring some form of identification such as a driver's license to register upon your arrival. There will be no refunds. Registration fee does not include hotel accommodation.


Cancellations: A $25 administrative fee will be charged per registration cancellation before March 1, 2009.
Absolutely no refunds for cancellations after March 1, 2009.
